A couple things I'd like to add to Linda's excellent suggestions:
1. We use a form to send home a month or so in advance to advertise the event and
solicit requests for autographed copies of one of the author's books. On that form
we ask for the usual information (name, class) but we also ask who to address the
signature to. This form is slipped into the books beforehand and ready for the
author. We return the same form to the recipient with the book just to insure that
there's no misunderstanding.
2.Many publishing companies will send copies of the author's books to your school
at a very good discount (Randon House and Simon&Schuster both discount 40% off
list) and with NO shipping charges. If you have left overs you may return them but
at your own expense. Check with the author as to the number of the publisher if you
book them directly.
3. Although we give a fairly advanced cut off date for the return of the
reservation form, there are always those who will come in at th elast moment asking
for signed coopies (usually teachers!). So we always order a few extra copies of
the books and have NEVER had to return one!

able to get a few new ideas from her list.
