Hi, Here's a different approach and one our district has used for years. We do not have permission forms. When students enter the district it is assumed that they have permission unless the parents complete a form to OPT OUT. In our 1300 student middle school no one has opted out and only one or two students opted out of having their photo used in press releases or on the web site. The opt out approach saves a lot of paperwork.
