[LM_NET] question

I am currently in a library program at Longwood
University and nearing completion.  One of my
assignments in my current class is to discuss
evaluations of library media specialists.  I was
wondering if anyone could forward me a form or explain
how they are evaluated by their principal.  If I could
compile a few evaluations, I could then see how
different levels (elementary, middle, and high) are
evaluated and make up one utilizing the data that I
find. Thank you.
