According to the 6th edition of the "MLA Handbook for Writers of
Research Papers" ---"If more than one city of publication are listed
give only the first".

I teach that you use the first one listed in your country.  Also, agree
that if it is a MAJOR city, and unique in name, you do not have to list
the state or country.

I was taught to use New York City if that was the first as that is where
most of the publishing companies had their offices.  Otherwise, use the
first major city if there were more than one.
