My school just purchased 30 brother laptop computers for student use.  Now
we are wanting to create a form for the students to fill out when they
want to take them home overnight.  If anyone on this list already has a
form they use I would appreciate it if you could send me a copy.  I've
looked at the one the local public library has, but some of the
consequences and procedures can't be used in a school situation.

If have the forms in a digital format, you can send them as an attachment
